Student Attendance

When submitting a request to excuse an absence or tardy, please include the following:

• Student's Full Name
• Date(s) of absence/tardiness and time frame if tardy (i.e. Yesterday(Sept. 20th) was 2 hrs late)
• Student's Teacher & grade
• Reason for missing school (i.e. home sick, family vacation, transportation issues)
